Chiefs, you can use the form to approve your Fire Fighters for the Fire Warden to pay for all class registration fees. See Directions below.
Chiefs, you can use the following forms to pay for the material fees for your Firefighters. Material fees will be the State Fire Training Certification fees.
Important Update – Starting Jan. 26, 2026
County Fire Warden will not longer be pay material fees that includes, SFT Certification Fees for students enrolled in COS classes. See attached Payment of Training Certificates 2026.pdf
Please complete this form prior to attending class to request payment from the County Fire Warden for your upcoming training. Fire Warden will cover Tuition and Certification Fee’s until Jan. 26, 2026.You will have to cove the cost of your textbooks.

The form must be signed by your Siskiyou County Department Chief.

Submit the completed and signed form to the Fire Warden before the class begins.
